      目的 探讨1999年-2018年美国国家健康与营养检查调查(National Health and Nutrition Examination Survey, NHANES)的心脏代谢指数(cardiometabolic index , CMI)与类风湿关节炎(rheumatoid arthritis, RA)之间的关系。方法 从1999年-2018年的NHANES数据库中收集数据。根据腰围、身高、甘油三酯和高密度脂蛋白胆固醇来计算CMI,RA患者资料来自问卷数据。采用加权多元回归和亚组分析探讨CMI与RA的关系。在此基础上,使用限制性立方样条(restricted cubic spline, RCS)研究非线性关系。结果 本研究共纳入27 063例关节炎患者,其中1 899例(7.0 %)患有类风湿关节炎。调整所有协变量后,多元Logistic回归分析显示,高CMI水平与患RA的风险相关(OR=1.815, 95% CI:1.441~2.285, P<0.001)。交互作用测试显示年龄存在交互作用。在RCS中,CMI与RA呈非线性关系,但这种非线性关系仍提示需控制低水平的CMI。结论 CMI与RA之间存在正相关关系。CMI是一种新颖、有价值且易于采集的代谢标志物,可用于预测成人患RA的风险。

      Objective To explore the relationship between cardiometabolic index (CMI) of national health and Nutrition Examination Survey (NHANES) and rheumatoid arthritis (RA) from 1999 to 2018. Methods Data was gathered from the NHANES database during the year of 1999 to 2018. The CMI was determined based on waist circumference, height, high-density lipoprotein cholesterol and triglycerides. The questionnaire data was employed to identify RA patients. Weighted multiple regression analysis and subgroup analysis were employed to investigate the connection between CMI and RA. On this basis, the nonlinear relationship was studied using restricted cubic spline (RCS) . Results In all, 27 063 patients were considered for this research, among which 1 899 (7.0 %) had RA. Following an adjustment for all covariates, multivariate Logistic regression analysis revealed that with a high CMI level, there is a larger possibility of RA (OR=1.815, 95% CI: 1.441-2.285, P<0.001). The interaction test showed an interaction influence of age. In the restricted cubic spline, a non-linear relationship between CMI and RA is shown, but this nonlinear relationship still indicated that we need to control the low level of CMI. Conclusion Generally speaking, it has been observed that CMI and RA are positively correlated. Our research demonstrates that CMI is a outstanding, paramount, and easily accessible metabolic biomarker that is able to predict adults’ RA risks.
