| 骨质疏松症（osteoporosis，OP）作为退行性骨代谢疾病的典型代表，以骨量进行性降低、骨小梁微结构退变为核心病理特征，其发生发展涉及遗传易感性、增龄性机能衰退及细胞能量代谢紊乱等多维度病理因素。中医学“脾肾相关”理论深刻阐释了骨代谢稳态的调控机制，共同维序“髓充-骨坚”的生理平衡。现代分子生物学研究表明，线粒体生物发生与能量代谢不仅是细胞衰老的标志性事件，更是OP病理进程中成骨-破骨细胞耦联失衡的关键调控枢纽，其生物能量转化能力衰退直接导致“骨失所养”的病机转归。目前脾肾“微观辨证”体系尚未建立，存在中医理论与线粒体功能的机制对接不够精准、线粒体动态网络与中医“整体观”融合不足等诸多问题，故本文以“脾肾相关”为切入点，结合现代医学线粒体生物发生与能量代谢的分子机制，系统解码“脾肾-线粒体-骨代谢”轴的中西医交叉科学内涵，为构建“补肾填精，健脾益气以促线粒体新生、以调能量代谢”的中医药防治OP新范式提供理论基石。 |

| Osteoporosis (OP), a typical degenerative bone metabolic disease, is characterized by progressive bone loss and trabecular microstructural degeneration. Its development involves genetic susceptibility, age-related functional decline, and cellular energy metabolic disorders. The spleen-kidney correlation theory in traditional Chinese medicine (TCM) reveals the regulation of bone metabolic homeostasis, maintaining the marrow-bone-strong balance. Modern molecular biology shows that mitochondrial biogenesis and energy metabolism are not only signs of cellular aging but also key regulation hubs in the imbalance of osteoblast-osteoclast coupling in OP. Their declined bioenergy conversion directly causes the lose of bone nourishment pathogenesis. Currently, the spleen-kidney microscopic differentiation system has not been established. There are issues like imprecise TCM theory and mitochondrial function mechanism connection, and insufficient integration of mitochondrial dynamic networks with TCM's holistic view. Therefore, this paper starts from spleen-kidney correlation, combining modern medical mitochondrial biogenesis and energy metabolic mechanisms. It decodes the TCM-Western medicine crossover connotation of the spleen-kidney-mitochondria-bone metabolism axis, offering a theoretical basis for creating a new TCM prevention and treatment model of OP, i.e., tonifying kidney essence, invigorating spleen qi to promote mitochondrial renewal and to regulate energy metabolism. |
