| 目的 基于ERα、ERβ调控OPG/RANK/RANKL通路探讨针刺防治绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)的作用机制。方法 选取60只SD雌性大鼠随机分为正常对照组(Control组)、模型组,采用去势术建立PMOP动物模型,验证造模成功后将造模后的大鼠随机分为模型组(Model组)、健脾益肾强骨针法组(Acu组)、雌二醇组(E2组)、健脾益肾强骨针法+雌二醇组(Acu-E2组),分别按照相应的针刺、E2药物和针刺联合药物治疗干预6个月。Micro-CT断层扫描感兴趣区域图像,分析各组大鼠左侧股骨骨密度(BMD)、骨体积分数(BV/TV)、骨小梁厚度(Tb.Th)、骨小梁数(Tb.N)、骨小梁分离度(Tb.Sp);HE染色法观察大鼠左侧股骨组织形态变化;酶联免疫吸附法(ELISA)检测大鼠血清中E2、卵泡刺激素(FSH)、促性腺激素释放激素(GnRH)、抗酒石酸酸性磷酸酶(TRACP)、Ⅰ型胶原C端肽(CTX-Ⅰ)的水平;逆转录聚合酶链反应(RT-PCR)测定下丘脑及骨髓细胞ERα、ERβ、OPG、RANK和RANKL mRNA水平;免疫印记法(Western blot)测定右侧股骨组织ERα、ERβ蛋白表达水平。结果 与Model组比较,Acu组、E2组和Acu-E2组能显著改善大鼠股骨组织的BMD和骨微结构参数(P<0.05)以及其病理变化;同时降低大鼠血清TRACP和CTX-Ⅰ含量,减少血清FSH、GnRH和增加E2含量(P<0.05);并且增加大鼠下丘脑和骨髓细胞ERα、ERβ和OPGmRNA表达水平,降低RANK和RANKL mRNA的表达水平。结论 针刺可有效防治PMOP,其机制可能与通过激活OPG/RANK/RANKL通路,对下丘脑-垂体-骨骼轴起整体调节作用,增加雌激素与受体结合的生物学效应相关。 |

| Objective Based on ERα and ERβregulation of OPG/RANK/RANKL pathway to explore the mechanism of acupuncture in preventing and treating postmenopausal osteopor-osis (PMOP). Methods Sixty female SD rats were randomly divided into a normal control group (Control group) and a model group. PMOP animal models were established using ovariectomy, and successful modeling was confirmed. The ovariectomized rats were then randomly divided into the model group (Model group), the spleen-kidney strengthening and bone enhancing acupuncture group (Acu group), the estradiol group (E2 group), and the acupuncture combined with estradiol group (Acu-E2 group). These groups received corresponding acupuncture, E2 medication, or combined treatment for six months. Micro-CT was used to scan images of the region of interest, and parameters including bone mineral den-sity (BMD), bone volume fraction (BV/TV), trabecular thickness (Tb.Th), trabecular numb-er(Tb.N), and trabecular separation (Tb.Sp) of the left femur were analyzed. Hematoxyline-osin (HE)staining was employed to observe morphological changes in the left femoral tiss-ue. Enzyme-linked immunosorbent assay (ELISA)was used to measure serum levels of est-radiol (E2),follicle-stimulating hormone (FSH),gonadotropin-releasinghormone(GnRH), tartrate-resistant acid phosphatase (TRACP), and C-terminal telopeptide of type I collagen (CTX-I). Reverse transcription-polymerase chain reaction (RT-PCR) was performed to determine m-RNA levels of ERα, ERβ, OPG, RANK, and RANKL in the hypothalamus and bone marr-ow cells. Western blot was used to assess ERαand ERβprotein expression in the right fe-moral tissue. Results Compared with the Model group, the Acu, E2, and Acu-E2 groups si-gnificantly improved BMD, bone microstructure parameters (P<0.05), and pathological ch-anges in the femoral tissue. These treatments also reduced serum levels of bone metabolis-m markers TRACP and CTX-I, decreased FSH and GnRH levels, and increased E2 levels (P<0.05). Furthermore, they upregulated mRNA expression of ERα, ERβ, and OPG in th-e hypothalamus and bone marrow cells while downregulating RANK and RANKL mRNA expression. Conclusion Acupuncture can effectively prevent and treat PMOP, possibly by activating the OPG/RANK/RANKL pathway, exerting a holistic regulatory effect on the hy-pothalamus-pituitary-bone axis, and enhancing the biological effects of estrogen-receptor bi-nding. |
