| 绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)是女性绝经后因雌激素水平下降,出现以骨量低下、骨微结构改变、易于骨折为临床特征的全身性代谢性骨病,具有高发病率和高致残率的特点。新近研究发现成骨细胞增殖分化功能以有氧糖酵解为主发生糖代谢重编程,维持骨髓微环境能量平衡,促进骨形成。骨髓微环境中成骨细胞有氧糖酵解通量与活性不足是诱发绝经后骨量低下的重要原因,该过程与中医学“肾痰”在骨的病机理论相契合。本课题组长期从事中医药防治PMOP的研究,本文基于“肾痰”在骨的病机理论,结合成骨细胞糖代谢重编程与PMOP的相关性进行论述,探讨从“肾痰”论治PMOP的内涵,进一步丰富中医从“痰”论治该病的科学实质,为PMOP中医药防治提供新的辨治思路。 |

| Postmenopausal osteoporosis (PMOP) is a systemic metabolic bone disorder that develops in women following menopause, primarily due to estrogen deficiency. It is marked by reduced bone mass, deterioration of bone microstructure, and heightened susceptibility to fractures, leading to high incidence and disability rates. The proliferation and differentiation of osteoblasts primarily depend on aerobic glycolysis for metabolic reprogramming. This process is essential for maintaining the energy balance of the bone marrow microenvironment and promoting bone formation. Insufficient flux and activity of aerobic glycolysis in osteoblasts contribute significantly to low bone mass in postmenopausal women, aligning with the pathogenesis theory of kidney phlegm in bone health. Our research group has been dedicated to studying the prevention and treatment of PMOP based on the kidney phlegm theory. This article examines the relationship between the glycolytic metabolic reprogramming of osteoblasts and PMOP, exploring the treatment possibilities for PMOP through the lens of kidney phlegm. We aim to enrich the scientific understanding of PMOP treatment from the perspective of phlegm, thereby providing new diagnostic and therapeutic approaches using traditional Chinese medicine. |
