| 膝骨关节炎(knee osteoarthritis,KOA)作为绝经后女性高发的慢性退行性关节疾病,其患病率显著高于同龄男性,而雌激素水平下降被证实是其发病的关键因素之一。雌激素作为女性体内重要的内分泌调节因子,不仅在生殖系统中发挥关键作用,通过多靶点调控机制参与骨-软骨复合体稳态、肌肉力学特性及炎症微环境平衡。绝经后,女性卵巢功能逐渐衰退,体内雌激素水平系统性下降,这一变化与膝骨关节炎发病风险显著上升关系紧密。该文系统阐述绝经后女性体内雌激素水平的变化以及与膝骨关节炎发病机制之间的内在联系,分析雌激素变化是如何通过关节软骨退变、软骨下骨改变、滑膜炎症反应和肌肉功能减退等机制促进膝骨关节炎发生与发展,并介绍相关临床干预措施,旨在为临床防治绝经后女性KOA提供全面的理论依据与新的研究方向。 |

| Knee osteoarthritis (KOA), a chronic degenerative joint disease that is highly prevalent among postmenopausal women, has a significantly higher prevalence rate compared to men of the same age. A decline in estrogen levels has been confirmed as one of the key factors contributing to its development. As an important endocrine regulator in the female body, estrogen not only plays a crucial role in the reproductive system but also participates in the homeostasis of the bone-cartilage complex, the mechanical properties of muscles, and the balance of the inflammatory microenvironment through a multi-target regulatory mechanism. After menopause, the ovarian function of women gradually declines, and there is a systemic decrease in estrogen levels in the body. This change is closely related to the significant increase in the risk of developing KOA. This article systematically elaborates on the changes in estrogen levels in postmenopausal women and the internal connections between these changes and the pathogenesis of KOA. It analyzes how changes in estrogen promote the occurrence and development of KOA through mechanisms such as articular cartilagedegeneration, subchondral bone changes, synovial inflammatory responses, and muscle function impairment. Additionally, it introduces relevant clinical intervention measures, aiming to provide a comprehensive theoretical basis and new research directions for the clinical prevention and treatment of KOA in postmenopausal women. |
