      目的 探讨Graves病伴胸腺增生(thymic hyperplasia,TH)患者血清钙水平升高及骨代谢加重的可能性。方法 本研究为回顾性队列研究。共纳入2019年7月至2024年7月期间在本院内分泌科确诊为Graves病的104例患者。根据胸部CT检查结果将患者分为Graves+TH组与Graves组,比较两组的基线资料差异,斯皮尔曼相关分析(Spearman's rank correlation analysis)分析血清钙与其他变量的相关性,多元线性回归分析血清钙水平升高的影响因素。结果 Graves+TH组患者明显年轻(P<0.001),血清钙水平升高(P=0.001),甲状腺球蛋白抗体(thyroglobulin antibody,TGAb)水平降低(P=0.011),甲状腺2度肿大的比例更高(P=0.006),重症肌无力的患病率更高(P=0.004)。在多元回归分析中,胸腺增生、β-胶原降解产物(beta-collagen degradation products,β-CTX)、碱性磷酸酶(alkaline phosphatase,ALP)和甲状腺肿大是与Graves病患者血清钙水平升高相关的显著因素,共解释血清钙水平30.1 %的差异。结论 Graves病伴胸腺增生患者的血清钙水平显著升高。胸腺增生、β-CTX、ALP和甲状腺肿大与Graves病患者的血清钙水平升高显著相关,提示胸腺增生可能与Graves病患者的骨代谢加速之间存在潜在相关性。

      Objective To investigate the possibility of increased serum calcium level and increased bone mobilization in patients with Graves' disease with thymus hyperplasia(TH). Methods This study was a retrospective cohort study. A total of 104 patients diagnosed with Graves' disease in the endocrinology department of our hospital from July 2019 to July 2024 were included. Patients were divided into Graves + TH group and Graves group according to the results of chest CT examination. The difference of baseline data between the two groups was compared. Spearman's rank correlation analysis was used to analyze the correlation between serum calcium and other variables. The factors influencing the increase of serum calcium level were analyzed by multiple linear regression. Results Patients in Graves + TH group were significantly younger (P< 0.001), serum calcium level increased (P=0.001), TGAb level decreased (P=0.011), the proportion of second-degree thyroid enlargement was higher (P=0.006), and the prevalence of myasthenia gravis was higher (P=0.004). In multiple regression analysis, thymus hyperplasia, β-CTX, ALP and goiter were significant variables associated with increased serum calcium levels in patients with Graves' disease, jointly explaining 30.1% of the difference in serum calcium levels. Conclusion Serum calcium levels were significantly increased in patients with Graves' disease with thymus hyperplasia. Thymus hyperplasia, β-CTX, ALP and goiter were significantly associated with increased serum calcium levels in patients with Graves' disease, suggesting that there may be a potential correlation between thymic hyperplasia and accelerated bone metabolism in patients with Graves' disease.
