In Traditional Chinese Medicine (TCM) theory, the principle of ‘ treating yin for yang ’ , emphasizing the interdependence and mutual reinforcement of Yin and Yang, serves as a critical guideline for treating osteoporosis with kidney deficiency and blood stasis pattern. With the accelerating aging population in China, the prevalence of osteoporosis continues to rise, particularly among patients characterized by kidney deficiency and blood stasis. The core pathogenesis lies in the imbalance of Yin and Yang in the kidney, leading to a pathological state of ‘ failure of Yang to transform Qi and Yin to shape form ’. Modern studies reveal that this Yin-Yang disharmony is closely associated with bone metabolism disorders, including osteoblast-osteoclast imbalance and differentiation imbalance of bone marrow mesenchymal stem cells. Through literature retrieval from databases such as CNKI and PubMed, classical formulas like Jinkui Shenqi Wan and Yougui Wan were identified. These formulas employ herb-pairing strategies (e.g., Fuzi with Shudi, Rougui with Shanzhuyu) to warm kidney Yang while nourishing Yin, achieving dynamic balance through the ‘ Yin-nourishing and Yang-promoting ’ approach. Compared with pure Yang-tonifying methods, this therapy not only avoids the risk of dryness-heat impairing Yin but also breaks the vicious cycle of ‘ kidney deficiency–blood stasis–bone deterioration.’This paper systematically elucidates the application value of the ‘ treating yin for yang ’ theory in bone metabolic diseases, providing a theoretical foundation for constructing comprehensive clinical strategies that ‘ regulate Yin-Yang, unblock blood vessels, and strengthen bones.’
