| 肌少症(sarcopenia)是一种与年龄增长相关的全身性骨骼肌退行性疾病,其病机核心在于气化枢机失司,脾胃、少阴、少阳枢机协同失常,导致气血生化乏源为本,痰瘀气滞为标,久之阴阳失济,肌肉失养不愈。现代研究发现,线粒体动力学障碍以致能量生成不足及自噬异常,是肌少症的主要病理机制之一,并与脾胃与少阳、少阴枢机不利之间存在内在关联。以“调枢复机”基本治则,辨证施治,可有效调控线粒体动力学相关蛋白,恢复线粒体内稳态,改善肌肉萎缩。故本文结合“气化枢机”理论与线粒体动力学,为肌少症病机理论与中医药辨治思路丰富理论依据。 |

| Sarcopenia is an age-related systemic degenerative disorder of skeletal muscle, with the core pathogenesis lying in the dysfunction of the qi transformation pivot mechanism and the consequent disharmony among the pivot mechanisms of the spleen-stomach, Shaoyin, and Shaoyang. This results in a fundamental deficiency in the generation of qi and blood, with phlegm stasis and qi stagnation as the syndrome of excess, ultimately leading to an imbalance of yin and yang and the failure of muscle nourishment and recovery over time. Modern research has identified mitochondrial dynamic dysfunction, characterized by insufficient energy production and abnormal autophagy, as one of the principal pathological mechanisms underlying sarcopenia, which is intrinsically linked to the impaired functioning of the spleen-stomach, Shaoyang, and Shaoyin pivot mechanisms. By adhering to the fundamental treatment principle of "regulating the pivot and restoring function" and applying syndrome differentiation, it is possible to effectively modulate mitochondrial dynamics-related proteins, restore mitochondrial homeostasis, and ameliorate muscle atrophy. Therefore, integrating the theory of "qi transformation pivot mechanism" with mitochondrial dynamics not only enriches the theoretical underpinnings of the pathogenesis of sarcopenia but also provides a novel perspective for traditional Chinese medicine syndrome differentiation and treatment strategies. |
