| 骨质疏松症是一种以骨量减少和骨微结构破坏为特征的全身性骨骼疾病,其早期症状隐匿,约50 %的患者在首发骨折后确诊。双能X线吸收法作为当前诊断骨质疏松症的“金标准”,往往因无法评估骨微结构而存在漏诊风险。高分辨率三维CT能通过亚毫米级(≤82 μm)的空间分辨率来实现骨小梁与皮质骨三维微结构的无创定量,为早期骨质疏松症筛查提供了新手段。本文系统综述高分辨率三维CT在早期骨质疏松症筛查中的应用价值,通过对比传统方法分析骨微结构参数与骨强度的相关性,并探讨其临床转化的机遇和挑战。 |

| Osteoporosis, a systemic bone disease defined by decreased bone mass and impaired bone microarchitecture, often presents with subtle early symptoms, leading to diagnosis in about half of patients only after their first fracture. While dual-energy X-ray absorptiometry (DXA) remains the current gold standard for osteoporosis diagnosis, its inability to evaluate bone microarchitecture may result in the risk of misdiagnosis. High-resolution three-dimensional CT (HR-3D-CT), with its sub-millimeter (≤ 82μm) spatial resolution, offers a novel approach for the screening of early osteoporosis by its non-invasive and quantitative assessment of trabecular and cortical bone microarchitecture function. This review systematically examines the utility of HR-3D-CT in the screening of early osteoporosis, explores the relationship between bone microarchitecture parameters and bone strength compared to traditional methods, and discusses the prospects and hurdles for its clinical implementation. |
