      骨质疏松性骨折(osteoporotic fracture,OPF)是骨质疏松症最严重的并发症之一,其防治需多靶点调控骨代谢失衡。中医药凭借其“多成分-多靶点-多通路”的整体调节特性,在调控骨代谢平衡方面展现出独特优势。本文系统归纳了近年来中医药干预OPF的分子机制研究进展,重点总结了其通过调控Wnt/β-catenin、BMP、MAPK、TRAF3、NF-κB、OPG/RANKL/RANK及AMPK/SIRT1等关键信号通路,在促进成骨分化、抑制破骨吸收等方面的作用机制。分析表明,中药活性成分可协同调控多条通路交叉网络,整合成骨-破骨-免疫微环境之间的对话,从而系统纠正骨稳态失衡。本文进一步提出,当前研究多集中于单一通路机制探讨,未来需借助网络药理学与新型递送技术,深入阐释中医药多通路协同调控的整体效应,以加速其临床转化与应用。

      Osteoporotic fracture (OPF), the most severe complication of osteoporosis, necessitates multi-targeted strategies to address the imbalance in bone metabolism. Recent research has highlighted the potential of herbal medicine in restoring the dynamic equilibrium between bone formation and resorption by modulating various signaling pathways, offering novel therapeutic avenues for OPF prevention and treatment. This review systematically summarizes the advances over the past five years regarding the molecular mechanisms by which Chinese herbal medicines intervene in OPF, with a focus on critical pathways such as Wnt/β-catenin, BMP, MAPK, TRAF3, NF-κB, OPG/RANKL/RANK, and AMPK/SIRT1. Evidences indicate that active compounds derived from herbs, such as icariin and astragaloside IV, may synergistically regulate cross-talk between these pathways, thereby integrating osteogenic, osteoclastic, and immunomodulatory processes to systematically restore bone homeostasis. We further conclude that current studies largely focus on isolated signaling mechanisms. Future research should employ network pharmacology and advanced delivery systems to elucidate the holistic efficacy of herbal medicine and accelerate its clinical translation.
